Missed and overdue test results are a leading cause of avoidable patient harm — and one of the most common, most defensible-looking failures behind a medico-legal claim. Not because anyone was careless, but because nothing in the practice was watching for the result that simply never arrived.

  • A biopsy is taken and never followed up. The pathology comes back positive — but no recall is triggered, and the result sits unactioned.
  • A referral is sent and the loop is left open. No one tracks whether the patient was seen, or whether the report ever returned.
  • Inboxes, sticky notes and memory are the safety net. When the practice is busy, silence looks exactly like "everything is fine".
Silence

Escalate on silence

If a result never returns or a loop stays open past its SLA, Sentinel raises it — quietly to the owner first, then up the chain. Silence becomes a signal, not a gap.

Draft the action

Sentinel drafts the human action that closes the loop — recall letter, re-excision booking, patient SMS, or claim — for one-tap approval. Nothing sends on its own.
